3. Can the sign answer basic questions? (Example: What month are we in, year, etc….)
4. Can the signer explain (written, spoken, etc..)what is being signed?
5. Does a family member try to speak for the signer?
6. Is the signer under heavy medication?
7. Is the hospital staff concerned about the signer notarizing documents?
8. Can the signer sign their name or is their signature by a mark only?
9. Does the signer have an I.D. card?  If so, what kind of I.D. card and when does it expire?

As always, I have to trust my gut feeling when notarizing documents.  If I feel that the signer is not fully aware and is being influenced by others, I have the right to refuse to notarize the documents in question.
